Clinical Pharmacy Support Designed for Safer & Smarter Long-Term Care
Texas Integrated Care Group provides integrated in-house clinical pharmacy services designed to improve medication safety, strengthen chronic disease management, and support better long-term health outcomes.
Unlike many traditional clinics, our care model includes direct access to a licensed clinical pharmacist as part of the patient care team. This integrated approach allows patients to receive more personalized medication support, coordinated treatment planning, and ongoing medication monitoring within the same practice managing their long-term healthcare needs.
Our clinical pharmacy services are led by Dr. Chelsy Akoro, PharmD, RPh, who works alongside Dr. Basil Nduma to help ensure medication management remains a central part of each patient’s care plan.
