Dr Basil Nduma Internal Medicine Physician

Basil Nduma, MD, MSHI

Founder & Medical Director,
Texas Integrated Care Group

Dr. Basil Nduma is an Internal Medicine physician and active hospitalist dedicated to the long-term management of adults with complex and chronic medical conditions. Through Texas Integrated Care Group, he provides comprehensive, coordinated care designed to help patients navigate fragmented healthcare systems, improve disease control, reduce avoidable hospitalizations, and maintain long-term health stability.

After years of caring for critically ill and medically complex patients in the hospital setting, Dr. Nduma recognized a major gap in care that often occurs after discharge — when medications change, follow-up is delayed, chronic conditions remain unstable, and patients are left navigating complex medical systems on their own. Texas Integrated Care Group was created to bridge that gap through a more proactive and connected approach to internal medicine, with a strong emphasis on continuity, care coordination, medication optimization, preventive strategies, and close longitudinal follow-up.

His clinical interests include chronic disease management, transitional care after hospitalization, cardiovascular and metabolic health, medication management, preventive care, and longitudinal care for adults with multiple ongoing medical conditions.

In addition to his clinical training in Internal Medicine, Dr. Nduma holds a Master of Science in Health Informatics and has a stong interest in applying technology and modern care-management systems to improve patient outcomes, communication, and continuity of care.

At Texas Integrated Care Group, the goal is simple: deliver thoughtful, evidence-based internal medicine care with a strong emphasis on long-term patient relationships, coordination, and whole-person health management.

Dr. Nduma serves patients across Conroe, The Woodlands, and the greater Montgomery County area through telehealth and coordinated outpatient care.

“The best way to prevent medical crises is to manage chronic conditions before they become emergencies.”

Dr. Nduma specializes in the management of chronic medical conditions that often require ongoing monitoring, medication optimization, and coordinated long-term care. His practice focuses on helping patients improve disease control while reducing the risk of complications, emergency visits, and preventable hospitalizations.

He provides care for adults managing conditions such as diabetes, hypertension, heart disease, chronic kidney disease, obesity, respiratory illnesses, and multiple chronic conditions requiring closer medical supervision.

By taking a proactive and preventive approach to care, Dr. Nduma helps patients better understand their health while supporting long-term disease stabilization and improved quality of life.

Personalized & Preventive Healthcare

Dr. Nduma believes that personalized healthcare is essential to achieving better long-term outcomes. Every patient has unique medical needs, health concerns, and treatment goals, which is why he emphasizes individualized care plans tailored to each person’s condition and overall wellness objectives.

His approach combines evidence-based internal medicine with preventive healthcare strategies designed to identify potential concerns early and support healthier long-term living. Through ongoing follow-up, medication management, patient education, and coordinated care, Dr. Nduma helps patients remain more actively engaged in their healthcare.

He is committed to building strong physician-patient relationships centered on trust, communication, and compassionate support.

Technology-Enabled Patient Care

As part of his commitment to modern and proactive healthcare, Dr. Nduma incorporates technology-enabled care coordination and monitoring programs to support patients between visits.

Through services such as Chronic Care Management, Remote Patient Monitoring, Transitional Care Management, and Behavioral Health Integration, patients receive additional support designed to improve communication, monitor chronic conditions more closely, and help identify health concerns earlier.

These programs allow for more connected and comprehensive healthcare while helping patients maintain greater stability and continuity in their treatment plans.

“The best way to prevent medical crises is to manage chronic conditions before they become emergencies.”
